• DocuSign: This form can be completed and signed digitally through DocuSign and will automatically route to our administrative team after signing.
• Printable PDF: This PDF can be printed and completed by hand. After signing, please scan or take a clear picture, then email as an attachment to studentsupport@ucdavis.edu.
